Registration

 

How can I register as a NHS patient?

 

To register with the Bayswater Medical Centre you must be resident within our practice area and have an identifiable purpose to remain in the UK such as work or study. Please visit NHS Registration to register online following NHS Registration Entitlement Assessment.

 

Please submit the registration form online once completed. A member of staff will contact you to arrange an appointment for your health check.

 

For your appointment you are required to bring in relevant documentation, as mentioned when you received your registration entitlement code as well as a health check. You will not be registered at the surgery without this documentation.

 

When attending for your first appointment, we would ask you to bring documentary evidence of your residential address and your right to remain in the UK.

 

What if I am living in the UK only on a temporary basis?

 

You are welcome to register at our surgery to see the Doctor privately.
