NHS PATIENT REGISTRATION
To register with the Bayswater Medical Centre you must be resident within our practice area and have an identifiable purpose to stay in the UK such as work or study. Alternatively, you must have a valid visa for more than six months to remain in the UK.
Please click on the link below "Apply now", which will assess your entitlement and if eligible, we will provide you with a registration entitlement code. The registration entitlement code needs to be entered onto the registration form for us to process your application.
Please submit the registration form online once completed. A member of staff will contact you to arrange an appointment for your health check.
When attending for your first appointment, we would ask you to bring documentary evidence of your residential address and your right to remain in the UK.
